A Step-By-Step Guide/Checklist

() Make sure your Passport is in
order...you will need one to go to Costa Rica.
()Schedule
a date for your surgery. Please
contact us for available dates.
() Take photos of yourself (for facelift
patients take close-ups of front and side views of your face) and send
them to Dr. Macaya by email at: fmacaya@arrivacostarica.com
() With your photos, please include a cover note
with your name, any questions and your preferred surgery date. You
will receive a confirmation of your dates.
() Fill out the
Health Questionnaire and
send it to us. You may send your photos by separate email.
() Make an appointment for your pre-op physical
(print out the "Preparations
prior to surgery" and take it with you when you go in for your
tests). Dr. Macaya prefers the test results to be done within a month of
surgery. The doctor prefers the blood and heart evaluation results
to be as close to your surgery date as possible. If you do not wish to
have the pre-op physical and test done at home, you may have them done here
at a minimum cost.
() Once
you receive your confirmation, you may make your airline reservations.
When you have your flight schedule set, please
email the Las Cumbres Inn with your
dates, and arrival time, and they will reply with a confirmation. They
will pick you up at the airport and provide all the transportation
during your stay.
